I-Pad printing instructions. Please read before you start to print.
To print this form from your IPAD you must follow these directions.
First click on the document this will make the form on the left bigger and will center it in your screen.
Then choose the print mode from your browser it is the square with the top arrow. (Top right of screen)
Then from drill down menu choose print.
This is important part. Make sure you change Scaling to 75% and change Range to 1 page.
Then click print.
